10/14 ~15 Peaceful days
這兩天腦子用得太多，一到五點就想把腦子關機停工。
這一個星期其實都很平靜，除了一些八卦以外。
但基於MSF policy（阿，好大的帽子！），恕不提供八卦詳情。
Matias需要正名，他叫Mathias，之前都少給他一個”h”.

這是我做的紀錄的一部份。
Already Done :
A. HR
2 Sterilizer: Santino Malek Ayet, Theresa Nyanut
2 Cleaner: 0730~1700 : Abuk Kuol, 1930 ~ 0500 : Theresa Ajok Tong (in Maternity) Between the gap (1700~1930, 0500~0730), Find a cleaner from Maternity
1 Laundry : (Name) share with Maternity
B. Log:
a. Electricity : Each side of the OT has electricity, the Anest. side has a stabilizer.
b. Baby warmer has already set.
c. Another 120L water tank has been set. But nobody fill the tank today.
d. Apron, gloves are given to Theresa Nyanut (in charge to teach the cleaner and laundrywoman how to clean and wash). Still need broom, squeezer, mop, detergent soap.
e. One spring filter has already being placed on the abandoned Anest. machine.
f. One spotlight work with lead battery.
g. Alex has done the measurement in OT.
C. Medical:
a. Stratos has confirmed the baby care setting. (Ambu, warmer, twin pump…)
b. Another Oxygen concentrator will be brought to OT and test.
c. Pre-op preparation notice has being put on Maternity ward.
d. Already have Morphine, Fentanyl, Diclofenic, still need thiopental.
e. Make sure all the instrument sets are complete, if they are not, try to complete it. And inform the coming unit how many sets are incomplete.
D. To be improved:
a. Make sure the staff work on time.
b. Wash gown, scrub suit, store in MSF place.
c. Re-sterilize the drapes.
d. Wait for OT nurse.
e. Make sure the cleaning and laundry job done by correct procedure. (They always don’t measure the amount of the water and chlorine tablet (wrong concentration). Maybe one more 120L water tank with 0.1% chlorine is applicable.
f. The inventory of OT is easy and difficult to be done. We only have sets. But there is one incomplete laparotomy set, only picture on the wall, no details (name of the equipments). Wait for OT nurse to check? Many instruments come from wards and OPD every day. Difficult to make inventory of the whole hospital.

今天接到Yak Yak的通知關於cleaner的事情。
Alex今天整天幾乎都在忙OT的事，昨天跟他說OT的人需要喝水，
請準備一台飲水用的filter，一早filter就出現在OT了。
全新，高科技的Baby warmer大木箱很快的就從倉庫移到OT門口。
還不到中午，Alex就把Baby warmer組裝好。
昨天史醫師說，若Oxygen concentrator若沒接在穩壓器(stabilizer) 上，
運作就會不預期的掛點。
所以我跟Mathias說OT需要一個穩壓器，
今天早上穩壓器就出現在Log辦公室的門口。(Magic Log!!)
的而且確，Oxygen concentrator皆在穩壓器上運作的非常好。
想起之前送回Log辦公室的那台concentrator，
是不是也是因為沒接在穩壓器上所以無法順利工作？明天搬回OT測試。
中午回去跟藥局拿了熟悉的鴉片類止痛藥，讓自己覺得心安。
由於OT需要幫醫院的各個部門消毒器械。
不僅Santino抱怨各個部門送來的器械並不完整，
各個部門也抱怨OT送回去的器械有錯，
有時候打開盒子裡面的器械並不是正確的組合。
所以昨天我決定，從今以後OT只提供完整組合的器械。
若各部門送回的器械有錯，或者有遺漏，OT保留不完整的器械。
並通知該部門找出遺漏的器械，而各部門之後也都傳來正面的回應。
而我希望能夠把MSF的刷手衣，手術袍都跟MoH分開。
由於之前沒有自己的cleaner，只有Santino一個人。
不知道叫誰去洗這些衣服。但有了自己的cleaner和Theresa，
只要把事情交代給Theresa，她就會要cleaner去做。
雖然不是完美或者非常主動的工作，但多注意，多叮嚀幾次，
她們就會把事情辦好。
缺點是她們做事情不太精確，關於消毒水的泡製都是大概大概，
沒有依正確的濃度去準備，所以未來的計畫是事先準備好正確的濃度，
讓她們直接取用消毒水來洗滌器械和衣物。
我嘗試把開刀房有哪一種器械，有多少，做出一個清單。
但因為我不是正職的OT nurse，器械的名稱充其量只認得六七成。
有很多器械真的長的很像，清點到Laparotomy set的時候我就放棄了。
2008年MSF目前不再有Laparotomy set，已經改成 Abdominal set.
即便有Laparotomy set的器材清單和照片，因為不認得每一樣器材，
所以清點到後來，有好幾個器械我們都不知道正確的名稱，
從Catalogue上印出的Abdominal set清單雖然有圖片比對，
但因為這兩個set並不完全相同，最後還是放棄，
只能等真正的OT nurse (Raewyn)來大概才有辦法清點。
而MSF的常見問題是，
每個expat的任期不一，有的三個月，有的半年，總之都會離開。
但Local staff幾乎一直都在這兒，若每一個expat來都給他們新的指示，
他們將會無所適從。
我想把一些布單和手術袍分開包裝消毒，方便使用。
但他們習慣全部包裝在一起消毒。
若只需要一兩條布單，打開這一大包東西，
其他沒用到的布單手術袍全部都要再消毒一次。
但我不想任意的改變他們做事的習慣，
只要他們可以把事情做好，是不是依循我的想法並不是這麼重要。
等OT nurse來，跟真正專業的人討論過再說吧。
傍晚史醫師來check了每一樣他照顧小孩需要的東西。
從Ambu bag, 嬰兒用的mask, baby warmer, suction, twin pump, towel…..
逐樣確定。
我相信，下一台手術來的時候，情況一定會比之前的好很多的。

坦白說我已經不記得發生什麼事情。
我開始跟Local staff學習Dingka，發音真是無敵難！！！
請看看標題，Peaceful days….忘記發生什麼也無可厚非吧！

Hmmm….
Pharmacy的Charles是Ivory coast來的好人。
我喜歡叫他的名字跟他打招呼，好像在叫自己一樣。
